This Coaching Agreement outlines the understanding between Root & Radiance Collective and clients who participate in wellness coaching programs and services. Please read this information carefully before engaging in coaching services.
Nature of Health Coaching
Root & Radiance Collective provides health and wellness coaching services focused on supporting lifestyle habits, mindset, and overall well-being.
Coaching may include guidance related to nutrition education, stress management, sleep habits, movement, and wellness goal setting.
Health coaching is collaborative and education-based, designed to empower clients to make informed choices that support their wellness.
Not Medical Care
Coaching services provided by Root & Radiance Collective are not medical care and do not replace treatment from a licensed healthcare professional.
Root & Radiance Collective does not diagnose, treat, cure, or prescribe for any medical condition.
Information shared during coaching sessions is for educational and informational purposes only and should not be interpreted as medical advice.
Clients are encouraged to consult their physician, nurse practitioner, or other qualified healthcare provider for any medical concerns.
Functional Lab Testing
Some programs may include optional functional wellness laboratory testing.
Any lab testing offered is intended to provide additional insight into general wellness patterns and is not used to diagnose or treat medical conditions.
Lab results may be reviewed in collaboration with licensed healthcare professionals when appropriate.
Interpretation and treatment recommendations related to lab results should be handled by a licensed medical provider.
Client Responsibilities
Clients acknowledge that they are responsible for their own health decisions and outcomes.
Clients agree to provide accurate and honest information during intake and coaching sessions.
Clients understand that implementing lifestyle changes, recommendations, or strategies discussed in coaching sessions is voluntary and at their discretion.
Clients agree to consult with their healthcare providers before making significant changes to diet, supplements, medications, or exercise routines.
No Guarantees
Root & Radiance Collective does not guarantee specific results or outcomes from coaching services.
Individual progress will vary based on personal circumstances, consistency, and commitment to recommended wellness practices.
Confidentiality
Root & Radiance Collective respects the privacy of clients and strives to maintain confidentiality of information shared during coaching.
Client information will not be shared with third parties without consent, except as required by law or in situations where there is concern for the safety of the client or others.
Payment Policy
Payment is required prior to or at the time of scheduling services, unless otherwise specified.
Fees for programs, sessions, and packages will be communicated clearly before purchase.
Prices are subject to change, and updated rates will apply to future services and new clients.
Cancellation Policy
Clients are asked to provide advance notice (for example, 24–48 hours) if they need to reschedule or cancel a session.
Late cancellations or missed appointments may be subject to a fee or forfeiture of the session, according to the policy shared at the time of booking.
Specific cancellation terms for individual programs or packages will be provided during enrollment.
Limitation of Liability
By participating in coaching services, clients understand that they are responsible for their own choices and actions.
Root & Radiance Collective and its representatives shall not be held liable for any direct or indirect damages that may result from implementing information or recommendations discussed during coaching.
Participation in coaching is voluntary and undertaken at the client’s own discretion and risk.
